You will be provided with an evidence log book, this allows you to gather evidence for all of the Learning Outcomes within the qualification as well as an additional log book for the practice hours which can be signed off.
During placement you must be supervised by someone at the same level or higher on a day to day basis, you will be observed and generate evidence which needs to be recorded. There will be a practice hours logbook to be completed which will allow the expert witness to confirm that you can replicate the required skill/procedure in the workplace to allow your assessor to make an assessment decision for each of the “be able to” outcomes.
Clear guidance will be provided to the expert witness on their role in the process. The record of placement hours should detail the clinical exposures of each period (shift) which can be referenced back to the outcomes in your logbook.
On completion you will have at least two reviews and a final assessment at the end of the placement hours with your assessor, any Learning Outcomes unlikely to be confirmed in the live environment can be signed off after confirmation of competency in an alternative placement setting or if permitted by simulation.
